Failure Rates

This is a zipped Microsoft Excel file of failure rate data for process equipment. The data are taken from a variety of published sources, all of which are referenced in the text. All the data assume the exponential distribution: i.e. failure rates do not change with time. A more comprehensive version of the file, including failure rates which are a function of time, is available from ility Engineering for a small fee. Click here to download.

PES Checklists

The UK Health and Safety Executive (HSE) used to publish a set of guidelines to the safe use of Programmable Electronic Systems (PES). These guidelines have now been superseded by IEC 61508. The original HSE guidelines contained a useful set of checklists which are largely unaffected by the requirements of IEC 61508 and are still issued as part of the material for the MSc degree in Process Safety and Loss Prevention at the University of Sheffield, a degree which is sponsored by both HSE and IChemE. As a service, we happily make these checklists freely available in the form of a zipped Microsoft Excel spreadsheet. Where appropriate, clauses in the checklists have been given explanatory comments. It is assumed throughout that the user is familiar with programmable control systems, the original PES Guidelines, and IEC 61508. Click here to download.
